Appointments

Being on time for your appointment is greatly appreciated to ensure you receive your full service and that other guests are not inconvenienced. Please arrive fifteen in advance of your appointment.

Cancellation

To avoid a cancellation fee, appointments must be canceled at least 24 hours in advance. Your card will be charged 50% of the service price for a same-day cancellation; 100% of the service price for a missed appointment without cancellation. To cancel or change your appointment, please call us at 312-925-6141

Pricing

We reserve the right to modify, discontinue, or raise prices on certain treatments without prior notification. This is to ensure that maximum standards of service and quality are met.

COVID-19 Waiver & Release Of Liability

D'Zhane Dollz take the COVID-19 pandemic very seriously and have put in place preventative measures intended to reduce the spread of COVID-19. In addition to our protocols, it is important to affirm our guests have not been exposed or have any symptoms related to COVID-19. By accepting/booking, you confirm:

• You have not been in close contact with or cared for someone diagnosed with COVID-19 at home or work.

• No one in my household (including myself) or workplace has experienced any COVID-19 symptoms such as fever, cough, severe acute respiratory illness, sore throat, loss of taste or smell and difficulty breathing.

• No one in my household or workplace has been in close contact with or exposed to any person(s) with COVID-19 symptoms and who is seeking a medical diagnosis.

• You do not have COVID-19 and have not been diagnosed with COVID-19 within the last 10 days.

• You are not currently subject to self-quarantine requirements.
